How do I apply for a U.S. Passport?

PASSPORT INFORMATION FOR THE CITY OF PLYMOUTH

Visit U.S. Department of State’s website for full instructions    www.travel.state.gov. 

New Applicants only (form DS-11) -All new applicants must appear in person

No credit/debit card payments.  Payments for U.S. Department of State can be in the form of personal check, money order, or cashier check ONLY. Payment for the City of Plymouth can be in the form of personal check, money order, or cash.  

New applications accepted Monday through Friday 8:00 to 3:30 pm.  No Appointment necessary

Complete IN FULL-Form DS-11, Application for Passport- in BLACK INK, no white-out or items crossed out. Do not sign application until instructed by agent.  Incomplete submissions will delay passport issuance.

  • Ø Include: Full Name
  • Ø Social Security number is Mandatory
  • Ø Date and place of birth
  • Ø Parents’ names, (Mother’s maiden name is Last Name at Parent’s birth), Dates, and Place of Birth
  • Ø Emergency Contact number
  • Ø Occupation,
  • Ø Dates of travel or N/A if not applicable.

Applicant(s) must provide: Proof of US Citizenship-

  • Ø Include: previous US Passport, Birth Certificate (mandatory for minors) or Naturalization Certificate,

Proof of Citizenship must be an ORIGINAL Birth Certificate with a raised seal, or Certificate of Naturalization.  Photocopies will not be accepted.  Original documents must be mailed with applications and will be mailed back to applicant after Passport Agency processes application.

Adult Applicant(s) must provide: Proof of Identity

  • Ø Current and valid Michigan driver’s license.  (If license is out of state, applicant must provide and additional form of picture id)
  • Ø Also acceptable are St of Mi ID card, Military ID card.

Applicant (s) must provide: One recent color passport photo.  Visit travel.state.gov for acceptable photo specifications.

  • Ø Applicant is allowed to smile.
  • Ø  Passport photos can be taken at: Costco, AAA, Kinkos, CVS, Walgreens….Snapshots, vending machine photos, school pictures, and full length photos are unacceptable.

MINORS

Children 15 and under MUST be present with BOTH parents.

Applicant(s) must provide: Form DS-11, original Birth Certificate, previous passport (if applicable) parents driver’s license, passport photo, check or money order

If one parent is unable to attend, a Form DS-3053, “Statement of Consent” form can be completed and notarized for absent parent.  Also required is a copy of the absent parent’s drivers license FRONT AND BACK. 

Children 16 or 17 must be accompanied by one parent.

Applicant(s) must provideForm DS-11, previous passport (if applicable), original Birth Certificate, drivers license or school id, ONE parent’s copy of driver’s license, passport photo, check or money order.

FEES- 2 separate payments

  1.  Passport Fee must be paid by check or money order payable to the U.S. Department of State

Adults-16 and over Passport Fee

Book     $110.00             Card     $30.00

Minors-

Book     $80.00             Card     $15, 00

Expedited Fee- $60.00 per application

Overnight- Return only- $20.66 per application

  1.  Processing Fee may be paid in cash or check payable to City of Plymouth

Adults and Minors- $25 per application
